This square sticky note is a simplified version of a DBT diary card, designed to make daily skill-tracking more approachable by literally making it into a tiny task.
This oversized stack of sticky notes (10cm square- or about 1” wider and taller than the most popular sticky notes) contains 50 sheets. Each sheet includes a check in on care/harm behaviors, a mood scale, and space to list DBT skills practiced.
